WIC certifications and mid-certifications

For a certification or recertification,

1. Proof of ID: drivers license, birth certificate, newborn crib card, immunization record
2. Proof of address: utility bill, phone bill, insurance bill
3. Proof of income: one month of paystubs, W-2 or taxes

For a mid-cert,

1. Immunization record

If the certification is for a child, they will need to be with you for this appointment.
